Other patients remain stranded in waiting rooms, ambulance bays, hallways and improvised treatment areas as their conditions deteriorate.

Triage becomes a final decision instead of an initial assessment. Abnormal results appear without reaching the person responsible for acting on them. Overcrowding divides responsibility until every task belongs to someone-but no one owns the patient.

Drawing from coroners' findings, government investigations, court records, official reviews and family accounts, Berg reconstructs what happened while carefully distinguishing documented conclusions from allegations and unanswered questions.
